Consent and consultation forms

Build the form a service needs, attach it, and customers fill it in before they come in. Start from a template or from the form you already use.

A consent for a lash lift, a consultation before a facial, an intake for a new client. If a service needs something signed or answered before the visit, build it once under Catalogue → Forms and attach it to the services that need it.

Three ways to start

  • From a template. Seven to choose from: a new-client intake, a general treatment consent, lash and brow consent, a skin and facial consultation, a massage intake, a hair colour patch-test consent, and a waiver. Every one is editable.
  • From the form you already use. Press Import a form and paste its text, or upload a PDF or a photo of the paper form. We read it into fields for you to check.
  • From nothing. Add the fields you need one at a time.

The fields

Short answer, long answer, yes or no, tick boxes, a choice list, a date, and headings and paragraphs for the words customers read, such as your terms. A paragraph can carry bold, italics, bullet or numbered lists and a link. Every question has a Required switch. A yes or no or a choice question can carry a follow-up: a question that only appears for one answer, so "Any allergies? If yes, which?" is two lines in the builder and one question for everyone who answers no.

The signature is always last and always required: the customer types their full name and agrees, and the time is recorded. Switch on Add a drawn signature too if you'd like one on the page as well.

Attaching it

At the top of the form, choose Every service or pick the ones it applies to. You can also attach forms from a service's own page, under Forms to complete first. A booking with two services collects both services' forms, and never asks a customer for one they have already completed.

Sending

Two switches on the form: send it when a booking is made, and repeat it in your booking reminders while it is still blank. The link rides the messages you already send, so nothing new goes out to customers who have signed. Filling in, signing, and where the answers live are the next part of this feature and arrive shortly.

What happens if I change a question?

Anyone who signed the old wording is asked again next time they book a service it applies to. Flipping a switch, such as the reminders, never asks anyone again.

Can I delete a form?

Forms are archived, not deleted, so anything a customer has already signed keeps its wording. An archived form stops going out with bookings, and you can restore it any time.
