Forms live under Catalogue → Forms. Press New form to start one.
Three ways to start
- From a template. Seven to choose from: a new-client intake, a general treatment consent, lash and brow consent, a skin and facial consultation, a massage intake, a hair colour patch-test consent, and a waiver. Every one is editable.
- Import a form. Paste the text of the form you use now, or upload a PDF or a photo of the paper one. We read it into fields for you to check. Anything we couldn't make sense of is named so you can add it by hand.
- Start blank. Add the fields you need one at a time.
The fields
- Short answer. One line. Choose what the answer must be: anything, a number, an email address or a phone number, and we check it when they sign.
- Long answer. A few sentences.
- Yes or no and choice list. Either can carry a follow-up: a question shown only for one answer, so "Any allergies? If yes, which?" is two lines in the builder and one question for everyone who answers no.
- Tick boxes. Any that apply, or statements to confirm.
- Date. A date of birth, a patch test.
- Photo or file. A patch-test photo, a doctor's letter. Up to three per question, 3MB each (15MB for a customer signed in to their account). Files go onto the customer's record, never into the email.
- Heading and paragraph. The words customers read, such as your terms. A paragraph can carry bold, italics, lists and a link.
Every question has a Required switch. Use the arrows to reorder, and the plus between fields to add one in the middle.
The signature
Always last and always required: the customer types their full name and ticks I agree, and the time is recorded. Switch on Add a drawn signature too for a box they sign by hand, with a finger or a mouse, which is printed on the PDF.
Attaching it to services
At the top of the form, choose Every service or pick the ones it applies to. You can also attach forms from a service's own page under Forms to complete first. A booking with two services collects both services' forms, and never asks a customer for one they've already completed.
Preview
Press Preview at any time, saved or not, to see the form exactly as customers will. It's also in each form's menu on the Forms page.
What happens if I change a question later?
Anyone who signed the old wording is asked again next time they book a service it applies to. Their earlier signed form stays on their record as it was. Flipping a switch, such as the reminders, never asks anyone again.
Can I delete a form?
Forms are archived, not deleted, so anything a customer has already signed keeps its wording. An archived form stops going out with bookings, and you can restore it any time.
